Register with the surgery

There are 2 registration forms for different circumstances in NI

  1. Patients wishing to transfer GP within Northern Ireland or transferring from Great Britain need to complete a form HS200.
  2. Eligible visitors or persons taking up ordinary residence in Northern Ireland need to complete a form HSCR-1.

Proof of address and Photographic ID will also be required at time of registration.

The below Nationals to shoe specific status and lawful presence in form of the following proof:

  • EEA Nationals:  Valid passport/ID, proof of address, maybe birth/marriage certs if needed.
  • NoN EEA Nationals (with visa):  Valid passport & visa/Bimetric Residence Permit (BRP).
  • Asylum Seekers/Refugee: Letter from Home Office, ARC card, BRP, or letter confirming refugee/humanitarian protection status.

You will also be asked to complete a new patient questionaire which will have information relating to your medical history and current medications. 

 

Accessing someone else’s information

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.

To requests proxy access, please collect a proxy access form from reception from 9am to 5pm.

Accountable GP

From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.

The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.

  • Patients do not need to see their named GP when they book an appointment with the practice.
  • Patients are entitled to choose to see any GP or nurse in the practice.
